Aged Care Advice & Tailored Support
$220 per hour including GST • One-hour minimum • Additional agreed time: $55 per 15 minutes
- Focused advice to understand your options and clarify aged care information.
- Practical assistance or appointment support within an agreed scope.
- An opportunity to meet Georgia and explore which Clear Pathway services or packages may suit you, with no pressure or obligation to purchase further support.

You have specific questions, need help with an agreed task or appointment, or want to meet Georgia before deciding on further support.
What’s Included
Scope, preparation, follow-up and estimated time are agreed beforehand. Comprehensive written plans, detailed provider research and implementation are quoted separately.
Package Credit
Up to $220 from the first paid appointment is credited towards a standard or tailored package purchased within 30 days for the same client. Applies once; additional hours and provider-funded work are excluded.
Comprehensive Aged Care Plan
$1,375 incl. GST
- A consultation of up to 90 minutes covering your goals, current supports, gaps and priorities.
- A review of any existing My Aged Care assessment information and approvals, with a plain-language explanation of what they mean, the supports approved and your next steps.
- A personalised written Clear Pathway Action Plan outlining relevant pathways and practical next steps.
- Your plan within five business days of the consultation and receipt of required information.
- One clarification email and an optional 15-minute follow-up within seven days.

You’re unsure where to start, concerned about changing care needs, or want a practical plan before support becomes urgent.

Additional Support
Assessment attendance, detailed provider research and implementation are quoted separately.
Assessment Preparation & Advocacy
$825 incl. GST
- Up to 60 minutes of preparation.
- Attendance at one assessment, up to 90 minutes, where authorised.
- A written summary and outcome review.
- A 15-minute follow-up once the outcome is available.

You have an upcoming My Aged Care assessment or reassessment and want help preparing and explaining your needs.

Assessment Advocacy add-on: $385 including GST, with scope agreed beforehand.
Care Implementation & Provider Navigation
From $1,375 incl. GST
- An initial appointment of up to 90 minutes to review relevant priorities and approvals.
- Research and comparison of up to three suitable providers to support an informed choice.
- A warm introduction once your chosen provider accepts the referral.
- Sharing agreed information with your consent and clarifying next steps.
Best Suited for
You understand the support you need but feel overwhelmed comparing providers and taking the next steps.
What’s Included
This package ends at the agreed handover.
Additional Support
Ongoing case management, additional provider searches and further coordination are quoted separately. Final scope and fee agreed upfront.
Complete Plan + Implementation
From $2,420 incl. GST
- The Comprehensive Aged Care Plan, including an initial consultation of up to 90 minutes and review of existing My Aged Care assessment information and approvals.
- Research and comparison of up to three suitable providers.
- A warm introduction to the chosen provider once the referral is accepted.
Best Suited for
You need both a personalised plan and practical help choosing and connecting with a suitable provider.
What’s Included
Before we begin, you’ll receive a written quote confirming what’s included, the expected timeframe and total cost. Support to prepare for and attend your My Aged Care assessment can be added by agreement for an additional fee. Ongoing case management and work beyond the agreed handover are quoted separately.
Booking & Confirmation:
Paid bookings require Georgia's approval to confirm scope, time, format, travel and price. Saturday appointments may be available by prior arrangement. Independent navigation means no guarantee of eligibility, funding, provider acceptance, availability or outcomes.
Travel & Costs:
Private travel is included within 20km driving distance of our Redcliffe base. Additional return travel beyond this area is quoted upfront at $110/hour including GST and vehicle costs. Parking and tolls are agreed separately.
Provider-Funded Support:
Provider-funded work is subject to a separate agreement and prior provider approval with no guarantee of funding.
