Service Description

From $2,420 including GST. Combines the Comprehensive Aged Care Plan with Care Implementation & Provider Navigation: an initial consultation of up to 90 minutes, a personalised written Clear Pathway Action Plan, research and comparison of up to three suitable providers, and a warm introduction to the chosen provider once the referral is accepted. The written plan is provided within five business days of the consultation and receipt of required information. With your consent, we share agreed information and clarify the next steps at handover. Before we begin, you’ll receive a written quote confirming what’s included, the expected timeframe and total cost. Support to prepare for and attend your My Aged Care assessment can be added by agreement for an additional fee. Ongoing case management and work beyond the agreed handover are quoted separately. Provider acceptance, availability, funding and outcomes cannot be guaranteed. Request to book; Georgia confirms suitability and availability before accepting. Private-client travel is included within 20 km driving distance of our Redcliffe base. Additional return travel beyond this area is quoted in advance at $110 per hour including GST, including vehicle costs. Parking and tolls are agreed separately. Provider-funded work is quoted under a separate agreement.
