Service Description

From $1,375 including GST. For clients and families ready to act on an existing care plan. Includes an initial appointment of up to 90 minutes, review of relevant approvals and priorities, research and comparison of up to three suitable providers, and support to make an informed choice. With your consent, we contact the chosen provider and coordinate a warm introduction once the referral is accepted, sharing agreed information and clarifying the next steps. This package finishes at the agreed handover; ongoing case management, additional provider searches and continued service coordination are quoted separately. Scope and final fee are confirmed before work begins. Provider acceptance, availability, funding and outcomes cannot be guaranteed. Request to book; Georgia confirms suitability and availability before accepting. Private-client travel is included within 20 km driving distance of our Redcliffe base. Additional return travel beyond this area is quoted in advance at $110 per hour including GST, including vehicle costs. Parking and tolls are agreed separately. Provider-funded work is quoted under a separate agreement.
