Service Description

Focused preparation and advocacy for an upcoming aged care assessment. Includes up to 60 minutes of preparation, attendance at one assessment for up to 90 minutes where authorised and agreed, a written summary, review of the assessment outcome and a complimentary 15-minute follow-up call once the outcome is available. Comprehensive Aged Care Plan clients may add Assessment Advocacy for $385 including GST. Clear Pathway is independent and cannot guarantee eligibility, funding or assessment outcomes. Private-client travel is included within 20 km driving distance of our Redcliffe base. Additional return travel beyond this area is quoted in advance at $110 per hour including GST, including vehicle costs. Parking and tolls are agreed separately. Provider-funded work is quoted under a separate agreement.
